Yes, this is 100% a true story of what we did to help treat a patient who came to the ER with a potassium of 8.4!
The patient was actually in the waiting room when we got the lab results back so needless to say, we got the patient into a bed and took action immediately. With the potassium being 8.4 (normal is 3.5-5), the patient was at an extremely high risk for potentially coding out in the waiting room, so they skipped a few of the stable patients in the waiting room to get a bed.
Here is a memory trick to help you remember treatment for severe hyperkalemia = “𝗱𝗶𝗰𝗸𝘀 𝗮𝗳” 🤪
𝗗 - Dialysis to help filter the blood of potassium
𝗜 - Insulin to help push potassium back into cells
𝗖 - Calcium carbonate to stabilize the cardiac membrane
𝗞 - Kayexalate to help poop out the potassium (some doctors will order kayexalate)
𝗦 - Sugar/dextrose to not bottom out the patient when giving insulin. You typically don’t do this step if the blood sugar is >250
𝗔 - Albuterol to help push potassium back into cells
𝗙 - Furosemide helps remove potassium through the urine
